Bad top surface quality
First thing i would do is dry the filament for a few 3-5 hours at 40 deg C, it clearly has moisture

Temperature tower curl/lift
if you have an encloser open the doors PLA likes to be in a room temp environment, closed chambers make it too soft

I don't have enough experience with idex but I believe the problem is somewhere in the Z offset/Home/bed tilt per tool head.
My friend on the other hand
Thinks if it's inconsistent then it's a mechanical problem.
