Mine is working again. Try shut down your printer, get him off and restart after a few seconds. Then reload the updater and then should be a klipper update there. After this all seems to be fine.

V4 idex z offset issue
Yeah this with the commissioning guide is a little bit confusing, you should start with the normal and switch then to the idex at the right point. I think it's not completely done but will be fixed in short time.
Hmm i think this will not have such a big effect. I had measure the extender and the nozzles and find a little difference to each other so I could fixed it. I hope you'll find the issue.

V4 idex z offset issue
I had an similar offset on mine, the only thing that helped was to change the nozzle extender between the hotends. Did you try this or are you using volcanos? I changed from over 0.1 like yours to 0.04.

4028 Lüfter
Schick doch mal deine printer.cfg rein.

#----------------------------------------------------- FAN ----------------------------------------------------------------
25KHZ PWM Signal: 0.00004 / 100HZ PWM Signal: 0.01 (cycle_time = 1 / pwm_frequency)
#--------------------------------------------------------------------------------------------------------------------------
[fan_generic part_fan_t0]
4-pin fan connected to 2-pin header on T0 (EBB42 v1.2) - digital pwm
pin: !toolboard_t0:PA0
cycle_time: 0.01
enable_pin: PA2
[fan_generic part_fan_t1]
4-pin fan connected to 2-pin header on T1 (EBB42 v1.2) - digital pwm
pin: !toolboard_t1:PA0
cycle_time: 0.01
enable_pin: PA3
