Diameter of printed cylinder too small despite calibration
It might be worth trying to calibrate at a larger scale? E.g. something 100mm long or longer. With a a small cube, small surface deviations - e.g. bulging corners due to pressure advance being slightly off - could throw out your calibration by quite a relatively large percentage when compared to corner bulging on a 100mm cube, say. I don't see any other reason for objects of a similar expected size coming out at different sizes when printed. As a sanity check, you could try printing the cube and cylinders at the same time just to confirm that your cube is indeed still 10mm as expected.

Layers not sticking past layer 3
It's hard to see in the photo, but is it possible that you are under extruding once you get past the first couple of slow layers? I.e. extryder can't keep up once print speeds increase causing stopping and starting of extrusion and peeling up of smaller sections. I had gaps in my prints from about layer 3 onwards. Initially resolved by slowing the printer - but then I noticed that the gear on the arm of the orbiter 2 extruder was very stiff. A bit of oil on this gear fixed things up. Might be a long shot, but worth considering I think.

Rough HG15 linear rails
No I didn't, but thanks for the suggestion. I did a few more rounds of tapping with a hammer and they have loosened up a little. But I think I'll give them a proper clean out and grease. Thanks!
