Updating from DH 2.1 to 2.3 server-side
Both have about the same amount of the world explored, but with different areas in some cases (Client A has an LOD that B doesn't and vice-versa, due to different cardinal directions being explored by each player). Any way to merge it?
If not then I'll pick the LODs of one client like you said

Acces violation crash when entering world
I've had the same error happen yesterday after a long time of not playing and on a new installation of my MC instance
The symptoms I've had were the CPU spiking to 100%, MC slowing down and then crashing (with DH complaining about CPU load in chat)
What resolved it was lowering the DH CPU load in mod settings, try it in the main menu before even entering the world (unless you already have it on the Minimal/Low Impact setting... at which point I dunno)
