Resource reload failed
The problem is directly related to the resource pack itself. If it's not, check if you have the CIT Resewn mod installed.

Crashing
From others' experiences, this may be a Minecraft problem, not an Iris Shaders/Sodium/Distant Horizons issue. Try again on your version in Vanilla.
