Persistent Holes, and Walls Between LOD and Normal Chunks
notice: I posted this in both "help-me" and "bug-report" because these things are bugs, and I want help to see if there are any fixes. If it turns out to be my error and there's an easy fix, I'll delete the bug report, but I didn't see anything that would fix this in the faq channels or previous "help-me" reports.
In the video I took, there are visible barriers between LOD and normal chunks; they're barely visible on land, but very visible in water.
Also another big issue: There are persistent holes in the world, which disappear when I go near, then reappear when I go back. It's impossible not to notice them, as you can see straight through the world when they're there.
In the video, I don't have any shaders or resources packs enabled, but my Minecraft client is loaded full of performance mods;
I attached the video in question, and a picture of my mods list.

for reference, here's each mod's purpose:
the holes are even worse with more LOD chunks rendered:
Here's another example of the border between LOD chunks and normal chunks:
I fixed the hole issue by saving and reloading the world, but the "walls" remained, though they're barely noticeable when custom shaders are applied.
Though neither of these issues should exist, I consider these fixed because they wont be noticed by me anymore, so I'll mark both posts as "Fixed/Solved"
fixing the walls in the water has been on james' radar for a while, but will probably be very hard to solve
How exacly shaders solve it?
It just not noticable but still there?
Or do they get rid of it?
