I switched all mine over to this format:
AUTH_PROVIDER: ldap
Not sure if it will be the same issue for you?

3 dcs is fine I think. I have other apps using it with same setup anyway. I'll install an LDAP browser and copy the paths straight from the directory itself and see what happens then.
