Some requests blocked by WAF?
Hope you don't mind me reviving this thread thread but I based my worker on a pastebin you shared ages ago (https://pastebin.com/B1UcWPbt) and I'm glad you made this post as I was confused as well!
Did you just change the http request to images to not pass in the headers of the original result? Then check it for status code 200 before saving to R2/Cache?

Fixes & issues
I made a fresh project following the docs and noticed that it has a higher version of
vercel installed.... which fixed everything...
