Suit for unusual measurements

I have very unusual measurements (shoulders on the large side of 54, waist ~50ish), so it's hard to find a suit off the rack. Even if pants/jacket are sold separately, the jacket is usually too wide around my waist. Should I keep looking for something RTW where it just works out (I'm looking for something relaxed, more on the casual side, so it doesn't need to be super tailored)? Or should I consider something MTM (suit supply?) – I'm hearing mixed things if MTM is actually beneficial in my situation. There aren't any bespoke options in my city and going to Italy a couple of times seems excessive for my first "nice" suit (I had some, but this is supposed to be one that I like). I guess I should first learn what works for me and what doesn't before I splurge on something custom. MTM or just invest time into finding nice RTW?

By 54 shoulders do you mean cm or size?

both size. so I usually need 54+ for the jacket to fit my shoulders, and 48-50 on trousers to fit my waist.

I’d take your body measurements and compare to some size charts. Also try on some jackets/suits and post the pics to a thread. Folks can help you a lot more with pictures than descriptions. Suit fit is difficult to get right

Others can confirm but I think Six inch drop (54 suit 48 waist) is pretty standard. The general advice is to fit the shoulders and length. Adjusting the waist (within reason) and sleeves can be done after by a tailor.
