spatie media library doesn't show the image

Hi. I used SpatieMediaLibrary Plugin in a filament page. it works fine and saves the image in the db but when I open the page the image is not shown in the form. I tried a lot of changes but it doesn't work. would you please check my file in this gist: https://gist.github.com/KiaBoluki/4399979809d8267c58df4a07ddd8b200 any comment would be appreciated.
