Seeing that you have V-sync enabled, it is likely that the option itself is the cause of your frame rate being unstable. Instead of using V-sync, try disabling it and set a fixed frame rate of 60.
In my testing, forcing the frame rate to a set value is far better at maintaining a stutter free experience than using Minecraft's V-sync option which is somewhat poor.
It may not eliminate your stuttering experience entirely, but it helps minimizing it.
