Yes, big database sizes are normal if you're on a bigger world.
What DH version are you using? Since 2.1, theres compression, which should reduce file sizes drastically

You can reset the LODs by just deleting the Database
Deleting the Database won't cause any problems, it will just delete all LODs
You could possibly make a script that deletes all LODs outside of a specific range, if you don't want to delete the LODs near your base
if you delete all LODs will it regenerate them the same way it did originally

- End: .minecraft/saves/WORLD_NAME/DIM1/data/DistantHorizons.sqlite
- Custom Dimensions: .minecraft/saves/WORLD_NAME/DIMENSION_FOLDER/data/DistantHorizons.sqlite
Multiplayer:
- .minecraft/Distant_Horizons_server_data/SERVER_NAME/
