why do i keep seeing buildings

None of the structures are real and disappear when i get closer

You somehow loaded LODs from a different world on the server
a modded world
its a local server

Yeah but you somehow have LODs from another world popping in
Try increasing multiverse similarity.
DH Settings > Advanced Options > Multiplayer > Multiverse Required Similarity %
i set it to 1
you will need to regenerate your LODs after doing that
you can do that by deleting the sqlite file(s)
/lodstore
Single Player:
- Overworld:
.minecraft/saves/WORLD_NAME/data/DistantHorizons.sqlite
- Nether: .minecraft/saves/WORLD_NAME/DIM-1/data/DistantHorizons.sqlite
- End: .minecraft/saves/WORLD_NAME/DIM1/data/DistantHorizons.sqlite
- Custom Dimensions: .minecraft/saves/WORLD_NAME/DIMENSION_FOLDER/data/DistantHorizons.sqlite
Multiplayer:
