DH 2.1.2-a-1.2.1 holes issue

Hi all, allow me to provide my mods list via SS. It generates weird anomalies (see attached) with holes in the world. The screenshot was taken with a render distance of 2. The problem is inconsistent (doesn't occur frequently, but happens in large groups), but does persist with logging out etc. This problem is sort of ignorable at high vanilla render distance like 32 chunks. Default DH settings for the version. Info: Multiplayer, Realms server.

Update: Shaders seem to break DH rendering :( (Complementary Reimagined, borderfog disabled)

(also check if your shader is up to date) The Swiss cheese problem can sometimes be attributed to a too low cpu load, which causes DH to not have enough time to convert the normal chunks to LODs
