I've been having this issue off and on for a week or so.
I've tried uninstalling and reinstalling SMM, including deleting left over files etc.
It did happen a few times with local mod installs too, but that seams to have stopped, only happens on the server now. Any help would be appreaciated.

uninstalling SMM wont do anything - its just a wrapper around downloading and placing mods.
Are you trying to install or uninstall mods on your local or on a dedicated server?
dedicated server.
do you have FTP access directly to the server?
Yes. That's what I've been doing, when this happens I just upload the file directly and it works fine, just wanted to make sure it was shared in case it a wider issue.
That's probably also why you're having more issues
However, it may be related to other things
@Mircea may want to take a look at logs to see if SMM can be improved here
Do you know what sftp server the server is running? It's reporting that the path exists, but then when trying to delete it it says that it doesn't
I don't know, the server is an AMP game server, if that helps.
