Invisible Video With Only Audio

Does anyone know why this happens? I can add controls to the video and play it normally, but I can only the audio come through instead of the video itself. I thought it was just chrome so I tried firefox and safari, but its still the same issue. Am I missing something? I just have it as <video controls src="video.mp4"></video> with no css or javascript. Any ideas would be greatly appreciated!
