How difficult is it to take paid FMLA leave for mental health? Does it require extensive doctor's note/recommendation?

FMLA is always unpaid. I think you're thinking of short term disability, which I don't think applies to mental health, only the physical manifestations of it if they occur
Companies may opt to provide paid parent leave, but that's wholly seperate from FMLA
