buying a shirt for tailoring?
If I'm going to get a shirt tailored because of my abnormal dimensions, how do I decide which size shirt to buy? Do I go off neck size? Chest?

Shirts are sized (at least at popular dress shirt retailers like BB and J. Press) by neck size and sleeve length. Fit is determined by different proprietary cuts (e.g. slim fit vs. traditional, Milano vs. Madison)
so get something that fits you in the neck and sleeve and get the rest tailored
Sleeve length can be adjusted, chest not really. If only they were sized by chest, other than vague faux-sizes like "slim" and "athletic."
Make sure it fits your neck, and is pretty good on shoulders and chest. Sleeves can be a bit too long but not too short, there's rarely enough fabric to extend them much or at all. Waist can be tailored to a lesser or greater degree, though several methods.
