Could you please help me with questionnaire for user interview?

I'm working on first project. I'm in the discovery phase, I did secondary research and based on that I wrote questionnaires for user interview. Could you please review it. I have lots of doubt, Did I eliminate Framing effect? Did I include necessary questions? Any mistake?
